There is a small room in back for tournaments, but all the cash games were up front when I was there. Sipping free Guinness while watching the world walk by on the Strip is about the best Vegas poker atmosphere I've experienced.
I played the $1-$5 spread limit and it was tougher than I expected. A couple very aggro players were there harvesting the fish, but there was money to be made there. A friend played the $1/$2 NL and said it was full of drunk, aggro college kids.
Dealers were fine, no mistakes and friendly enough.
The tables are right by the bar, and I was getting my next drink brought to me without even asking. They weren't wearing fancy outfits, but one was pretty hot. Easy 5 rating here.
Not really "managed" up front- you just sit down if there's an open seat. I did go back and chat with the manager for a minute though, and he was competent and friendly. The $5 rake on the $1-$5 spread game was ridiculous, so they lose points for that.
I didn't ask about comps but a player at the table with a player's card seemed to be having a hard time getting checked in.
